Change treatment goal "Name" field to "Goal" to match print out

On the treatment plan when determining the goals instead of having “Name” it would be helpful for it to match what prints out on the form which is “Goal”. I was so confused and tried to ask for so long about this matter. Now I know better.

Enter custom relationship to client when "Other" is selected in "Relationship to Client" dropdown on Contact Log entry. This would allow us to enter the most relevant option for this client.
