In the state of AZ, we are required to have both the client and therapist’s signature on all intake consent forms. It’s great that both are added to the treatment plan, but it would be great if it could be added to the consent forms. I always have to download it and re-upload it with my signature after the fact.

Arizona law R4-6-1101.5 requires the behavioral health provider sign and date the consent for treatment documents signed by the client. Please add for the provider to be able to e-sign and date intake forms.

As a behavioral practice, we often have times we need to include a clinician's signature on something that the client has also agreed to (dynamic forms). This helps display the collaboration and agreement of both parties outside of a treatment plan. It would be helpful to have the option of including a space for the clinician to sign on dynamic forms

Dynamic forms need to be able to be signed (as witness) by theranest account user (counselor, etc). Some forms require counselor or witness signature in addition to client signature.

This is a requirement in WA state under RCW 18.225.100! Right now all of our providers are having to print out the forms to sign then. Please add the ability for the provider to e-sign intake forms!

Our Licensing Board, AZBBHE, requires a dated signature on informed consent documentation. Since moving into telehealth and having my forms online through the client portal, I cannot affix my own signature to the forms. It would be helpful to add a "review and approve" signature to the portal intake forms to aid in compliance with our board requirements.
