Add rendering provider NPI and supervising provider name and NPI to statements and superbills
For a client to be able to file for out of network benefits, the statements and superbills need to have the rendering provider NPI listed and, if the therapist is an associate level therapist, the supervising provider's informaiton also needs to be on there. Please add these to the forms.

The new client statements now include NPI for both rendering and billing providers.
BCBS has started to request we include both the NPI for the Billing Provider AND the Rendering Provider on the Superbill, and they deny the claim without it. We checked in and they said it was new for 2018. It would be great if the Superbill had it for both. Thanks!
