Select which diagnosis code appears on the progress note

When completing a client's progress note, allow me to choose which diagnosis appears on the note so that the diagnosis code displayed reflects the treatment performed for that date.

    The progress notes page looks like a mess with every goal and objective and diagnosis. I agree, let us choose which one we are working on. Thanks.

    It would be great if we could have the Diagnostic Codes for the client in a check-box form instead of automatically generated so we can select different primary/secondary/treated conditions on each progress note we submit.

    I think the primary dx (focus of tx and billing) should appear at the top of the list and therapists should be able to edit and toggle the order of the list from session to session. This is how I’ve seen it work in other systems. It doesn’t make sense to house/edit dx on an “initial” assessment page bc an “initial” dx indicates “initial,” as in “intake”, whereas the current/actual dx may change over course of tx. The only way I’m aware that I can update dx is by changing it on the initial assessment page.

    Yes, please do this! It is required by insurance to list the diagnosis that was addressed in a session as primary on the progress note, and this must match the CMS1500 form. Currently it is extremely burdensome to go to the initial assessment tab, delete all dx's, then relist them just for a single session. TherapyNotes has ability to switch dx's into the primary field on every progress note; can you do this too?

    It would be most helpful to have a drop down of the client's assigned diagnosis available for each note, within the note (not case folder). Most of my client's have more than one diagnoses. I do not always know which diagnosis is going to be addressed when the client comes in for a session. For example: If a client has the diagnosis of PTSD, Depression, AND GAD in some session GAD and depression may be addressed, in another, just depression, in another just PTSD, AND yet another can address all the above. The way your system is set up, I have to chose which diagnosis (es) is going to be addressed before I can begin a note. I often, have to start a note with one diagnoses for the session, then change if after the client leaves my office. This is a problem for me and happens 8 out of 10 sessions a day. It would be wonderful to have a case folder for the client, then have available a drop down menu of the client's diagnosis for each note, so that it could be selected from at the end of the note.     My practice and I find it difficult when we must begin a note in order to have the diagnostic codes print on the superbill. We understand that all diagnostic codes may not be applicable to every service and the purpose is to match the correct diagnostic code to the correct service; however, starting a note before printing/emailing the superbill makes the billing process much longer. It would be extremely helpful to be able to choose which diagnostic codes to include when creating the invoice, this way superbills can be printed/emailed sooner.
