Display co-insurance in left sidebar when viewing a client's details

When viewing a client's details, display their co-insurance for their primary insurance in the left sidebar so I can quickly view that information without going to another tab.

      add co insurance rate option under Bill To & Insurance Info just like you have co pay amount listed.
