Allow org admins to unlock approved progress notes

As an org admin, allow me to unlock progress notes that have already been approved in case further edits need to be made. Similarly to how I can remove signatures from signed progress notes.

Automatically add supervisor to Provider(s) field in each progress note

If the staff preference permissions require a supervisor must approve or co-sign all notes, then automatically add the supervisor to the Provider(s) field of the progress note.

Additional signature lines on treatment plans

Allow for additional signature lines on treatment plans. For our clients that have medication the provider prescribing the medication needs to sign the treatment plan as well.
