Create an EMDR Processor Assessment as a standard note template

My counselors use EMDR in our practice and it would benefit us if Theranest provided an EMDR Processor Assessment form as option in Notes. I've attached the blank form below. I know Simple Practice EHR offers it.

Clear all Mental Status entries

Require linked appointment when saving Progress Notes

Have the contact log on the same tab as the progress notes
