Add billing information to the Detailed Attendance report

Allow me to view some billing information, (e.g., the amount that was billed, amount paid, etc.) when running the detailed attendance report.

Allow org admins to set a staff's calendar preferences

The director should be able to set calendar preferences for staff. It would be much quicker for one person to be able to set this up than training and waiting for each staff member to do this on their own.

Set default rendering provider for self-pay clients

Allow me to set a default rendering provider for self-pay clients so that the rendering provider can appear on invoices. Right now I can only set a default rendering provider for clients that use insurance.

Show planned interventions from last note on appointment block

When viewing a client's appointment on the calendar, allow me to see the Planned Interventions or Plan information that was entered on the last progress note.
