Archive Custom Forms

Add the ability to take an old custom form out of circulation without deleting all the same type of custom forms that have been filled out and are filed in active consumers charts

    Dynamic Forms can now be archived! Archiving a form allows you to remove the form for new clients, but the information will still be retained for existing clients. Check out our help article for more information: https://theranest.zendesk.com/hc/en-us/articles/360059286152-Archive-Dynamic-Form

    Get rid of this clutter! We need to still have access to old custom forms but don't need them in the same spot as the active form. THANK YOU !!

    Would love to be able to "ARCHIVE" a dynamic form vs deleting it. That way all previous information that is on those dynamic forms are not deleting, and still able to be accessed should they ever be needed. We use these dynamic forms to create assessment reports, progress reports & discharge reports.

    Being able to archive an old custom form without deleting info from client files would be MUCH APPRECIATED. Or at least provide the option to no longer have the forms appear as tabs on the notes page.

    Would absolutely appreciate the option to archive a custom form, so we can always go back and access the forms created within that custom form. However so it also is no longer showing up on the client portal. Also would love more features on the custom forms... almost like a word document to be able to have different fonts, have the repeat of the same word (example: "notes'" in more than one location on the form). Also be able to edit the note within the custom form after signing it, that way a whole new one isn't having to be created or copied just to edit one small area.

    Please create a function to archive a CUSTOM FORM. That way we do not corrupt older charts. And we get rid of the awful clutter from having too many versions of a form.

    Please do work one this feature as soon as you can. We are a nonprofit that has to amend our process and documentation on a regular basis. The need to archive custom forms is essential to eliminate confusion of which are the latest forms to fill out for our therapists.

    As I update my paperwork from when I first started, I would love to be able to archive old paperwork/intake forms so that they are not mixed in with the new paperwork. I do not want to delete them though in the event I need to reference them, but simply archive them so I still have them available to reference if I needed them (rather than deleting) This would make life so much easier!

    It would be very helpful if a therapist can make a previously used custom form inactive so it no longer shows up on the tab for future use. Currently, we had created a new and more efficient form and are using that one. We would love to remove the option for the old form, but doing so removes all of the gathered data from previously signed notes, which is quite unhelpful and unethical. Being unethical is extremely problematic. Can you please create the option to make a no longer used custom form INACTIVE, so it becomes dormant and is no longer able to be seen as an option in future charts? Thanks much.

    Please allow to archiving forms - I have created forms that no longer serve a purpose and/or need to be updated or changed in some way and am unable to do that - and I can't afford to lose forms that have already been filled out.

    I just asked TECH Support. Got an answer back that states this can not be done and you will need to go into each client file and download their forms and then upload to clinical documents to be saved separate. Then go in and delete the custom form. Can anyone say HEADACHE?

    Please do this! The option to do a quick upload to general documents would be perfect, as even making changes to custom forms deletes responses from previous clients. This is huge a liability for an agency. Unfortunately we assumed this was a feature (didn't think that Theranest would actually DELETE signed client forms) and realized after the fact that that data is just gone. Signed client forms not being able to be deleted (or at least not en masse) seems obvious and intuitive for any medical or mental health profession.

    The need/reasoning behind the request: Forms can (and often do) become obsolete when insurance, Medicaid, and/or Medicare rules change. The obsolete forms should remain as part of the patient's medical record and should be able to be accessed to be read. Currently if we change a custom form the changes are universal and change previous documents. It is not ethical to tamper with documents that have been signed by clinicians and clients. Those documents should be accessible in the patient's medical record in "read-only" format. Having obsolete form that are similar in form and purpose but should not be used accessible sometimes becomes confusing for some staff. Suggested change/update: Have and option to retire a form where it still can be accessed in a "read-only" format. New documents will not be allowed to be created on the retired form. A reminder can be posted at the top of the page that shows that this form is a retired form and to check with administrators for the current form. Have an option to "un-retire" the form accessible to administrators.

    This feature is needed! Laws change and forms need to be updated and when you have hundreds of clients it would take forever to download and upload a form for every client in order to update it and make changes. Either allow archiving it without removing completed forms or allow one button upload to general documents of each client. PLEASE! We a simpler way to stay in compliance!

    Or, if there could be an extra button/option that said something like "Upload copy to General Documents" or "Upload copy to clinical documents" so that we had the option, that would be really helpful. A lot of times, these forms are clinical/liability related and to have to manually upload each one is a time-consuming pain!

    Yes, this is a serious limitation to using custom forms. The only work around I've figured out is to "Microsoft Print to PDF" to save it, then upload to General Documents. I feel like it wouldn't be difficult to change this functionality. Since Theranest has added an automatic upload of signed intake forms through the client portal, there should also be an automatic uploaded copy of whatever form was completed if it was electronically signed.

    Agreed!!! We have custom forms for notes specific to our center. Often they become obsolete or changed vastly and we don't want to lose that data entirely, but we also don't want to have 50 tabs in our notes section. :)
