Definitely concur with other users - I need to be able to preserve the record of the original date and time I signed a note - and preserve it as it is- as well as record date and time I added an addendum, when I realize I forgot something I meant to include. Glad to see this is started!
I suggest the ability to update existing treatment plans instead of writing new ones by adding an addendum that allows us also to update the diagnosis if warranted within the treatment plan.
Please add this feature! The way it is currently designed makes us hardworking therapists look a lot sloppier than we are. For example, if I make edits to two of three of a specific clients notes, it makes it look like I've written all three on the same day. In terms of being audited or even providing note to clients, this would make my note writing look far more suspicious than it is.
I was happy to see that an addendum can be added to the initial assessment. I agree w/the others that it is very important that we become able to add them to progress notes, as well.
I would love to be able to add progress made after 120 days after treatment plan was made. This would be able to address it and have it attached to the old treatment plan before starting a new one.
allow an opption to alter or finalize session times. If a session ends later or sooner and the parent signs the session time can not be altered. which means the appointment can no tbe directly sub,itted for billing. it is making it dificult to use the billing feature if i can not submit the time or i have to for go the signature feature which is very necesassry.
I would like an addendum option added to custom forms. I use a custom diagnostic assessment and would like to make additions or note errors after I have signed the form.
Just wondering why a treatment plan becomes "Immutable" once a provider signs it. In my view, treatment plans can and should change over time. It would be nice to be able to sign a treatment plan, print it for the file, then change it later if/when appropriate.

Is there a way to not remove signatures and if I need to update the note I can just add an addendum? The auditing company is stating that this could be a liability that a therapist can potentially edit a note.
Is there anyway to take this out or update your system to only allow addendums without the removal of the signature?
Please update this system to allow editing/deleting of erroneous treatment plans! This is getting really ridiculous having several incorrect treatment plans in our clients' files. It is very messy!
Can an addendum option be added to notes so that the signature does not have to be removed as this makes it look as if you are doctoring documentation if you go back and remove and resign?
There should be a way to correct, amend, update diagnoses so that the initial diagnosis is not eliminated. I would recommend having a way to do this in the note and then having that update be reflected in any invoicing.
