Select which intake forms to send in client invitation

Add ability to select which intake forms will be sent to different clients when they are invited to the client portal.

      Our office works with children, adults and couples which all have different intake forms. It would be great to be able to separate the forms so that new clients did not have to weed through to fill only the forms out that applied to them.

      I too would love to be able to send out 'select' forms per client, as a child verses an adult intake form is quite different. Additionally, I take self-pay clients and some clients that have insurance pay all of their fee. So some clients would need to sign a financial form as I utilize Ivy Pay and some would not. Please let me know if I can talk with someone further about this being an added addition to Theranest. Thank you

      I desperately need this as well!! My practice sees individuals and couples at different stages. It would be great to be able to allow clients to say if they are coming for individual or couple counseling and then a different set of questions or given to the client. I think other EMRs are able to do this, so it would be great if this EMR was able to offer a feature that competitors offer.

      I work with children, teens, and families. In the mental health field, an intake questionnaire is drastically different based on the client's age. I have currently been sending the adult intake questions to all of my children and teen clients as that is my only option. It is very awkward to have to continually explain to new clients (who are just getting to know me) that I use adult questions for their child clients (questions pertaining to drug use and sexual activity/relationships for a 6 year old just doesn't work). I need to be able to select which intake documents I send to certain clients based on their ages. This is just not working for me and my practice and I'll have to find an alternative EMR soon.

      We're able to send whatever intake for we choose to different a specified clients at one time.

      We have different intake forms for early childhood, child-adolescence, and then adults. We also have additional forms for group (group confidentiality). Not being able to send a client intake and a child intake within the same day is one of our BIGGEST frustrations and barriers to recommending theranest to other practices like mine.

      I now have to send a client specific instructions via email how to complete the portal intake docs since I have different questionnaires for individuals, couples, families and minors. It is confusing for clients. Please add a feature, like exists in Simple Practice, to specify for each client which intake docs should be visible.

      Absolutely, clients currently have to fill out our 15+ page intake forms for EACH clinician. So they unnecessarily must go through about 6 of the same packet but for different providers. And there's nothing that separates minor intake forms vs adult intake forms.

      Please separate the intake forms so that clients can select only the necessary ones.

      Separate the intake forms and have patient be able to select the necessary forms for that patient rather than have to fill all out as they seem to be attached altogether.

      I’m sure this has been suggested, but I wanted to know if there was any plan to set up seperate intake forms for clients to complete. For example I have one set of paperwork for adults and one for minors. It’s not optimal that I can only choose one population to utilize the client portal as I do pay monthly for it.

      Can you allow for the option to use/remove certain portal documents for each client (e.g., no need to have an individual client see the family questionnaire, or see a minor consent form when they are an adult). Also, it would be helpful to include a customized message to our clients when they first login to the portal (i.e., a greeting or instructions for how to fill-out our forms).

      I think it would be great to select which forms go to which client because, for example, we have an intake form for parents to complete if the client is a minor. Currently client portal requires all of our clients to complete that form just to move forward. This becomes tedious and extra paperwork for no reason.

      YES! The forms and intake process needs a great deal of improvement. Currently I cant use the forms feature because it does not offer this functionality.

      I would really, really love this feature. I have created intake forms for adults and children. It will be confusing for clients to have access to all of them. I would like to be able to assign certain intake forms to clients. In addition it would be nice to assign a client a release of information form when it is needed and have them be able to sign it via the portal so that they wouldn't have to come into the office. Thanks so much :)

      Yes!! I really really need this function too! I have forms for career counseling clients, and forms for therapy clients. I always have to ask one client to wait while another fills the others out to go back in and re-lock the right forms into place (so much work!). Can't wait to see this feature come to pass. Thank you :)

      I would love to be able to have various intake forms for different providers. If one provider sees children, and another sees adults, it would be nice to specify which intake forms are needed for a certain provider and not have ALL intake forms show up for ALL clients.

      Allow clinicians to select different intake forms based on the client (e.g. an intake form for couples might differ from an intake form for an under 18 individual client than from an adult individual client).

      I really really need this feature. So that the following scenario can happen: client 1 is able to complete forms A, B, and C while client 2 only completes forms A and B (and not see or have access to C).
