Set default forum for appointments

Allow me to set whether the forum will default to Telehealth or In-Person when creating a new appointment. I only conduct sessions via telehealth, so I need to change this every time I create an appointment.

      Agree: it makes at least as much sense as default diagnosis or service code. Helps avoid careless errors when new visits get entered, especially conflicts if tele modifier needs to be set

    Set default Forum when scheduling Appointments

    Add an Organization setting to set the default Forum (In-person or Telehealth) when scheduling new Client or Group appointments.
