To make creating forms easier, multiple dropdown options created in the form element should be able to be selected at once. Or multiple checkboxes in one section. If I have asked a client which substances they have used in a form and there are 20 options, I can't simply list 20 dropdowns (because the client would only be able to check one at a time) but need to insert 20 form elements. This is WAY too much work.
Provide an option to add a multiple choice question in form creation. The checkbox works but it might take less time to be able to add all choices in some area. Attaching an example.
It is very difficult to make a form using checkboxes. For example, to make a symptom checklist, users have to add each item as an individual text box. If we could import a list via a spreadsheet or create a single item (like the dropdown feature), that would save some time.
In general, there needs to be significant improvement in the ability to send assessments, self-report measures, and forms to be signed. But this would be a start.

Create an option when developing Dynamic Forms that works the same as the pre-defined options of Mental Status and Risk Assessment so that therapists can create their own pre-defined options. For example...when using the Dropdown option in a progress note - I can enter as many options as I wish; however, when actually doing a progress note...I can only choose one of the options in the Dropdown when I actually need to choose several of the options; not just one. This would work amazingly well for choosing the interventions used during the therapy session instead of having to type out every intervention used.
Please create a "form option" on the dynamic form creation page that allows you to create a custom section like the set up of the "Mental Status Exam" premade option. That would allow us to make a custom section to choose more than one thing. Currently I have created a progress note with a lot of check boxes and when it prints its like 4 pages long- too long!

It would be nice to have a customizable check list of interventions on the progress note within the "Therapeutic Interventions" box that will print in narrative form depending on which interventions you checked. Customizable would be necessary as every therapist uses different interventions. This would save a lot of time to check a box versus writing the interventions out each time.

For example say I want to add a question to a form but want to also prompt my client with "you can have more than one answer". As of right now that is not possible.
