Include location in email when appointment is scheduled

Include the location information when the automatic email is sent to a client when an appointment is scheduled. Otherwise, they don't see the location until the appointment reminder is sent.

When adding or editing insurance to a client and specifying who the client's insurance was last verified by, allow me to also note the date it was last verified.
