Add organization email to intake forms

I would like to be able to add my email to the header of all my intake forms that I send out to all new clients

I would also like the header to include the option for the fax # since I have an ROI that we will be sending/faxing to other professionals. It would nice if they could see the Fax# on top.

If a custom form needed to be printed the only way for it to meet state requirements for clinical documentation would be to have the clients name on every page, date on every page, and page numbers. It would be great to have this control for our custom forms.
