Measure treatment success rate and track outcomes in a report

I'd like to have a treatment summary after treatment ends. I need a way to see the success or failure of client treatment, including outcomes based on specific diagnoses and treatment protocols (i.e. service types/CPT codes).

      On the PHQ 9 and the GAD 7, have you considered the following?: Adding the scoring rubric would be so much more helpful - seems like a simple link could be added.... Being able to see a graph over time of the scores for progress....this also would be a really nice feature.

      With the compelling research that is coming out about the effectiveness of the Outcome Rating Scale and the Session Rating Scale, TheraNest should do an integration with a company like MyOutcomes so therapists can track progress, the working alliance, and have the data readily available in each client's chart in TheraNest.

      Yes, a form that we can send out to all active clients through the portal where we can make questions that will help us understand how we are doing from a clinical standpoint as well as a admin standpoint. A good example is the MHSIP 6 month survey. If there was a way we could send that out and then track the data from those surveys for the entire practice.

      Yes, it would be nice to track clients goals and have an outcome percentage for each one.

      It would be great to have a way to track outcomes for clients and is standard to have a treatment summary. This change would be supportive to our organization.

      In order to note completed goals, a new treatment plan needs to be made before closing a case. It would be good to not need to create a treatment plan, but just note achievement in the closing case. Additionally, I would like to have a report for funders of our non-profit of outcomes in treatment plans. This could be just noting goals that are met or if a goal was made during a set report period.

      When closing a case, there is only one fillable field in the Discharge Summary (unless services are pregnancy related). Ideally, the discharge summary should list each treatment goal with a text box for progress toward that goal. The simpler technical fix would be to have a text box for free form notes under each section of the form. If/when the option to enter a paragraph of progress toward each goal is created, it would also be very useful to create a "treatment review" form that uses this same format. Treatment reviews are required documentation and currently not available in Theranest.

      Hello. I was wondering if you could add the statment "Medically necessary and appropriate to the recipient's diagnosis and needs" near the signature line of the treatment plan. We recently went through a routine records audit with Magellan Health and that was one item my counselors didn't have on their treatment plans. Instead of writing by hand on every treatment plan, my counselors requested that this disclosure statement be added. The Magellan rep who did the records review stated that it is a requirement for the treatment plan of all Florida Medicaid recipients. Thanks for your attention and consideration in this matter.       OH YES. I love that idea. I often give these. Another option I suppose would be to create a complete new note for that. Not sure how that would work out.

      YES! The fact that Theranest doesn't have a data/outcome tracker puts it behind other EMR systems.

      It would be helpful to generate a report based on client diagnoses that is similar to the reports offered for demographics reports.
