Remove demographic fields in TheraNest & Client Portal
Allow me to remove some of the demographic fields that are displayed in TheraNest and the Client Portal. There are some fields we don't need to capture (e.g. Religion).
Having both these sections feels redundant. Why not give Household Info all the same fields as Contact Network, with a checkbox to select whether this person is a client contact or not, and still have the ability to invite them to the client portal? As it stands, we use Contact Network almost exclusively and don't enter any info in Household Info because it is effectively redundant.
Also, Emergency Contact should be a part of this feature, rather than being a separate set of fields. This way a client could add multiple emergency contacts (for instance, adding both a child's parents)

This is huge. We need to be able to customize this to fit the info we need/want to collect and to eliminate the items we do not need. It adds to client confusion we there are irrelevant fields here.
YES!!! Please can we remove some of the extra demographic fields...these were clearly designed for hospitals, large facilities, managed care and not for the majority of mental health providers. The amount of information you have as default info should really be in the clinical fields is desired.
This might well be a deal-breaker for me. I see comments on here going back more than 2 years and nothing has changed. First, I wouldn't have even realized there was a demographic form had I not signed up on my newly created account as a test user. That's unacceptable. Secondly, there is no way to edit the demographics questionnaire, and it is the FIRST FORM MY CLIENTS WILL SEE. Also unacceptable and will cause me to lose business.
I own a speech-language practice that only works with children. I do not think that the demographic questions are appropriate for children and I do NOT want to gather that information. I would like to make that an optional part of each client's portal. I have had several complaints from parents and I don't want to lose business. Thank your for your advice.

I would like a way to eliminate some of the demographic info requested when sending the invite to the client portal. I don't feel comfortable asking families for their Social Security Number (something I do not need) or their toddler's sexual orientation.
When I send an invitation to the client portal, my client is asked to fill out their social security number, sexual orientation, gender, etc. I know they can leave those items blank, but I would prefer them not to be on the form at all. Can you make it so we can remove those items please?
I would prefer to remove all these fields and have clients write in gender, race, sexuality, etc rather than select from a limited drop-down list. Please allow us to remove/edit these fields.
The demographic information that is preset is too involved. My understanding is that after sending a Client Portal Invite, and client signs up, and before they review the forms that we require - the Demographic Questionnaire can't be changed by Manager/Administrator of the TheraNest program.
Please change functionality to allow administrator to either
---quick fix, turn it off entirely
----or, allow the administrator to turn off certain questions.
PROBLEM is...
--there's too much personal information being asked at a point that is too early in the client/therapist relationship.
---The on-line setting for disclosing this information is too impersonal.
-- we have 6+ other forms that we need them to fill out/pay attention to that are more important that we don't want them burning out before they get to them
--yes we can use the "Client Portal Invite Message" to message "it's OK to skip any non required questions", but we'd rather use verbiage for focusing prospective client attention elsewhere.

The option to totally customize the demographic questions both presented to clients on the portal and in client details would be ideal (along with ways to control whether the client can input/edit particular items), but at the very least, I absolutely do NOT want to ask people's religion or SSN !
I searched to see if I had missed some settings somewhere to fix this, and then searched here to find out if someone else had made this suggestion. I'm discouraged by the fact that it was initially suggested in 2017. This is a major reason why I have not gotten around to using the client portal feature in the past. It really turned me off when I first saw what the clients would be asked and looked like it would be unhelpful.
If if won't be changed, I'd like the form instructions to say that not all providers need all of this information--leave blank and check with your provider if you aren't comfortable answering particular items.
Also, I think it's kind of out of date to have home vs mobile phone blanks (it should be phone #1/ preferred number, and phone #2/optional--along with whose phone it is--i.e., my personal phone, my work number, my husband's)--and can I text you at this number?

YES! It would be great to remove fields we don't use, and also to disable the client's ability to view or change demographics fields. I agree with what everyone's said here already, and I want to add another important note:
Some of the demographics fields are fields that populate the CMS claim form. These fields need to exactly match what the insurance company has on file in order for claims to clear. Currently, because the clients can edit their own demographics info, a client can cause claims to be rejected because they've put a different first name, last name, or gender than what the insurance company has. It's a massive problem because it makes it harder for us to get paid properly. It would be a huge improvement if we had the option to toggle both hide/show fields and enable/disable editing of certain fields in the client portal.
Agree with all these... at least allow Organization Admin to customize both what fields are on the Client Portal demographic form and what options are in the drop-down list for those fields.
I would like the ability to change what demographics clients complete in the client portal. I am uncomfortable asking clients for their religion but cannot remove it.
I would love to see a fully customizable demographic form that can be created by the organization, with the ability to enter or remove fields, as well as make certain fields mandatory or not.
