When my treatment plan is up for review, allow me to write a review summary for the current treatment plan rather than editing the old one or writing a whole new one.
It would be so amazing to have a Treatment Plan Review document, or allow for a space to review the current treatment plan without having to reinvent the wheel.
The treatment plan serves as the review. You just need to start a new document and it auto-populates the last treatment plan. Then under one box i name it PROGRESS UPDATE and put the narrative there. You can edit goals and dates as well. I dont see how a separate review form would be any different.
I agree with all of the above. I had to create my own template but it is still a lot of extra work and isn't linked with the treatment plan as it should be.
there is only a reminder to review the plan, but not to actually write a review. Also, if you sign it, you can't then make changes to it. If you don't sign it and make changes to it, those changes go back into historical notes and add those changes everywhere.
Treatment plans require regular review in order to be compliant with specific insurance plans. There is no current form to do this effectively.
I don't feel a treatment plan made the custom form creator will appear as organized as the current form.
Would it be possible to add a treatment plan review template or possibly allow customization of the current treatment plan form?
Yes, it is a lengthy process to create my own quarterly review document, copy/paste the current treatment plan and all of the required information when a couple boxes could be entered to a new treatment plan form (please have it auto-populate from the last treatment plan) which can be edited. Boxes needing to be added would be:
1. Review of progress
2. changes to goals
3. recommendations/discharge planning based on review
4. Date document reviewed with client
5. Client signature
6. Staff signature
7. Supervisor signature if applicable
There need to be a way to review and update treatment plans without creating an entire new treatment plan and making notes extremely long. Please update this feature...PLEASE!!!
Develop a progress review form to use when it is time to review progress on old treatment plan. Then have a new clean treatment plan with only the new goals and objectives on it and have this only be attached to the progress note. Old treatment plans need to die when goals have been achieved. Having them still attached to notes is too unwieldy and wastes paper if you were to print them out. I often avoid updating treatment plans for this reason.
If we were to have a field where we could set the treatment plan review reminders for specific date ranges after the initial session date, that would be rather handy. For example, we could tell it that reviews need to be done at 30, 90, 120, and 270 days after initial session. Then it would remind us at that time to make sure we have it done by our required timeframes.
The ability to transfer the Treatment Plan entered in the a Treatment Plan tab in client information and as we create a case have the ability to choose from that treatment plan already entered.
or
Have a list of treatment plans we can generate on our own and apply to the case we create through an easy selection.
I would love for this suggestion to be considered and implemented. For me the treatment plan is a living document that is updated and revised as treatment progresses. It would be sooo nice to open up the treatment plan and see all the updates, revisions, and comments about progress or lack thereof in one documents. As it is set up now I find it really confusing and there doesn't seem to be a way for me to reflect what progress has been made.
Develop a way that the treatment plan can be originally signed, re-opened without removing original signature so that it can be reviewed, a place to make comments, and then on the same treatment plan offer review signatures versus having to develop a new treatment plan. Offer review reminders every 90 days.
