Would like the ability to format custom forms similar to other word processors:
- Adjust font sizes, colors
- Paragraph styles
- Inserting tables
- Adding images
Allow an image to be uploaded to intake forms. We would like to add the option for the client to add an image of their insurance card to our intake forms.
Also PLEASE PLEASE allow drop-menus created to be collapsed when working to edit the form. If I have 20 options in a drop menu, it is so frustrating to have to scroll past them all to get to another element I am creating.
Also-also, please make it so we can move around the order of the drop menu lists without having to cut and paste to move the entire list up or down to accommodate additions to the list. It would be much easier if we could drag and move them around, if possible.
The spacing between sections of documents is atrocious and results in a significant waste of paper when hard copies of documents are needed. There is no need to have 2-3 inches between each section of a SOAP note.
The formatting for the forms has : pre-populated. We need these removed, sometimes we will need to add a different punctuation.
Two columns do not work for entire forms, and having three pages of check boxes takes up too much space.
Allowing tables to be inserted would allow three columns of check boxes to take up less space and look much better.
When speaking to the person selling this service (Theranest), I was told that the forms could be customized further to our liking with all of these adjustments without any problems soon as we subscribed - only to find out I had to create a suggestion. :)
We also need the clinician's signature to be auto-populated into the forms as we can the client's name. Some of our forms need to be sent to court-ordered caseworkers, and we have to use a completely different system for these forms. We would like to be able to have more functionality to reduce the number of systems the agency has to use to complete all the daily tasks. Thanks
This is still very much needed! Being able to edit or format documents in a way that you specifically need them displayed (ex. column splicing for only part of the document rather than the current "all or nothing" format) would be incredibly useful.
It would also be helpful if there was an option to make drop down menus a required field as needed. As it stands, client's can skip the drop down menus when completing intake forms because there's no option for drop downs to be set as required.
The appearance of forms is unprofessional and hard to read. Formatting options, or at least a global update to font and sizing, would be immensely helpful. At the very least, please ensure that the size of the responses is smaller or same size as question/prompt and that they are the same font. It is embarrassing as is.
We are in the process of creating Intake forms for use in the client portal and are disappointed in the lack of formatting options. Please add more advanced formatting for fields such as an enhanced text editor or CSS/HTML support.
Would like the ability to format dynamic forms using similar features as Microsoft Word and Excel. Also having the ability to grab other information from the EHR i.e., pulling in treatment plan information into new dynamic forms.
It would be really great to be able to have conditional options within a progress note. For example... a yes or no question. If the answer is yes - it would populate the follow up questions that goes along with the yes answer. If the answer is no... there is not need for additional information.
Agreed that forms look super unprofessional and should have easy ways to include extra lines without having to create a whole new element on the form, as well as easy formatting options for font styles. It's kind of embarrassing how bad the forms look.
I love some of the suggestions, however, my biggest need is to simply improve the line spacing and ensure that font is sized consistently. Presently, any questions that exceed a single line of text are crammed together without adequate space. Additionally, the spacing between the questions and the answers are inconsistent. Finally, the question text and answer text are different font sizes. This doesn't offer a polished or professional look for our forms.
The fonts and visual presentation of the forms looks clumsy and unprofessional when printed. Please allow for stylist preference with more latitude in generating formats, fonts, and personalization.
