Title of submitted intake form to be titles of forms instead of shared form

I noticed when we have Patients fill out forms through the Portal, after they finish, the completed forms in the Patients documents are labeled “shared form” with a number after. Is there a way to alter the naming convention of this either after or before? It’s a pain to not know which document contains which info.

    The title of an intake form could not be included in the file naming convention for the stored PDF versions of the completed form because the form title field in the app allows for special characters (&*%$# etc.). If an intake form title included a special character in the title, it would potentially break the file if those characters were inserted in to the PDF file name.
