Add the ability to enable multifactor authentication for the Client Portal for portal signups and when logging in to add an extra layer of security for both clients and practices.

Though we are prompted for an Authentication Code at login, it's actually not necessary to login.
For example, if I try to login, when prompted to enter the Authentication Code, I do not enter it, open a new tab, go to the login screen, type in my username and password, I'm allowed entry into my account without an Authentication Code.
