Don't select note type if only one available

Remove the dialog box that has us select a progress note template. We only have one progress note template available for our whole practice so it's adding an unnecessary step.

      The pop-up dialogue box makes it so we can't open several progress notes in the background from the missing progress note page, without re-generating it after each note. It also takes several seconds to load and adds an unnecessary step that has slowed down our workflow significantly.

      It would be great if organizations that set a default note type could do away with the "select a note" box once the default is selected company wide.
