For everyone who says it can't be done, I recreated our agency's assessment form as a dynamic form and haven't had any trouble pulling forward information re: diagnosis, dates, name and information so long as I dragged the correct boxes (in bold type) over to the form. I used paragraph boxes for each heading and it was easy to do. It shows in the notes tab with all our other forms (I made them the same way). A little time consuming but it's done.
Allow us the ability to do multiple assessments in one case. Sometimes, a diagnosis changes or there arises a need for a more thorogh assessment and an addendum is not the most appropriate or helpful way to document this form. Insurance does allow for additional assessments to be billed for when medically necessary and being able to adequately document our assessments would be helpful. Currently, creating our own form does not automatically pull in the diagnosis. If we could modify the assessment form that TheraNest has created that would be amazing. Or if we could copy what TheraNest has made and then add to it, even better. Always keeping a copy of the original. However, being able to add more than one assessment in one case would be very beneficial.
Would love the ability to customize the Initial Assessment and create additional ones as well. We ask for different information during intakes for adults than we do for children. Also, working with children in the child welfare system necessitates we have much more data than a child in their biological home.
It would also be great if we could add a field for Rule Out Dx.
Allow us to create a customized assessment that functions like the one created by TheraNest. Currently, we can design our own, however, it does not link to appointments, nor does it pull in a diagnosis, etc. Those can only be done by using the default forms created by TheraNest. Having the ability to either modify the default forms build by TheraNest, or the ability to create our own would be extremely beneficial.
Just adding my voice to the chorus that it would be great if we could customize the Initial Assessment form. At my practice, we are creating a separate Progress Note template to use instead, but it would be great if we didn't have to do that.
I totally agree, If I could pull forward Dx, Tx and other information, I would love to create my own assessment report, this one is a little light on the required data. Thanks for considering.
A specific spot for the diagnostic impression would be great, and one that is separate then the rest of that tab. My staff get confused and then fill out the whole form, and then have to fill out our dynamic diagnostic form as well that we require. I'd love to be able to archive that whole form to avoid the confusion.
CCA needs to be customizable to allow it to be completed according to Medicaid standards. Would also be great to be able to have a CCA specific to a child/minor.
Would love to be able to customize to Physical and occupational therapy as well, many fields are unrelated. Making this customizable would be a huge benefit!
Current initial assessment is incomplete. It does not provide biopsychosocial and MSE as mentioned by the previous commenter. Please allow us to customize the template. This is important when we bill state insurance.
The native initial assessment lacks a lot of fundamental information fields relevant to psychotherapy and state compliance. Please make them customizable or add important fields like substance use and addiction history/factors, physical/medical history/health, identities, trauma history, sex/relationship history/factors, MSE, etc. and probably a miscellaneous field too. Your assessment does not meet minimum state guidelines for compliance.
