Make it easier to specify whether completed intake forms should save to clinical or general documents

I have said this before elsewhere, but please allow us to send an initial client questionnaire (an intake document that has clinical information in it) directly to clinical documents instead of to general documents with the administrative forms. This is a serious confidentiality issue. I believe it is a legal liability for providers and for TN itself. I know there have been comments about this since at least 2020 and no apparent action by TN.

I can't believe this concern has not been addressed since it was brought to TN's attention as far back as 2020. Also, if I could create the initial client portal account myself, then I could go ahead and send clinical intake forms/questionnaires to a client before the first session. Please, please address this TN!

In my practice I have an initial phone consult with a client during which we usually schedule a first appointment (why would I wait until they make a portal account to schedule an appointment?). I then send the client portal invitation email with intake forms. I have decided that I do not want my client questionnaire (in which the client describes their history and symptoms in detail--which saves a lot of time in the first session and helps me with documentation) to be saved in the general documents, since billing staff have access to general documents (that's where I put a copy of the front and back of their insurance card). I want to limit the exposure of my clients' sensitive clinical information, just like with progress notes. I would like to be able to set up the client's portal account myself (perhaps after that the client could sign in with and OTP process instead of a password--perhaps more secure anyway). That way I can send them the clinical intake questionnaire right away. Or it would help if I can send the questionnaire with the other intake documents but specify that it goes to clinical documents to be saved there. Either way would work and would make my clients' sensitive information more secure.

All of the above comments! I'm currently NOT using the portal for my intake form and any assessments that I would like a client to complete, because 1) they all save as one file rather than individual files and 2) they all go to general documents rather than clinical documents, which is where they belong. I don't use dynamic forms for these because then they can't be changed or deleted without losing prior data AND I don't want them to show up for every clinician in our practice. If we all used dynamic forms for everything, the forms at the top would take up half of the screen.

Would it be possible for development team to add an option for where each company would like the returned pdf documents to be returned. This option could be under 'Action', 'Edit' and have either the General or Clinical docs be set as default so they at least come back somewhere. In addition, it would be beneficial to have each individual form sent through the client portal to also come back as an individual pdf document, not grouped all together (i.e. if I send 3 forms, I should get 3 pdf's returned, not 1). This would eliminate having to send multiple emails (and clients receive multiple emails) just so we can group the forms sent through the portal by how we want them grouped after they are completed. This would also be HIPAA/PHI compliant as there is no way of moving a document between the folders without first downloading to your computer or network.

I agree, I think it is important that we are able to distinguish between general and clinical documents that are shared with clients and that are then signed and submitted to the chart via this process. It is not efficient to have to save to computer and then upload again to the clinical side. In this new age of more telehealth, it is critical that theranest develop means for efficient sharing of forms and a more efficient means of REVIEW AND SIGNING OF TREATMENT PLANS (not yelling here just want that to stand out in this comment). I have had to try to develop work around forms that I can send to the client via the portal that they can sign (since currently this seems to be the only means that theranest has to allow clients to sign documents via telehealth) indicating that they have reviewed and signed their treatment plan. I also document this in the progress note for that session. However it would be more efficient to share screen and have them sign or send via the portal. A work around of downloading the document to a computer, uploading to an email attachment, sending email to client (via theranest) is not efficient and leaves documents downloaded outside of the EHR on other devices to achieve this goal (potential security risks that are not necessary).

It would be helpful to change the completed forms that a client completes in the Client Portal to be automatically downloaded in to Clinical Documents instead of General Documents so that nonclinical staff, who have limited access in Theranest, would not be able to view them since they would not have access to Clinical Documents.
