Add "Start Time" and "End Time" labels to printed progress notes

Add labels to the time that is displayed on the printed progress note, such as "Start Time" and "End Time" so that it's clearer to payers.

    Hello for insurance claims we need to have a place in the progress note that we can update the time accurately to how long the session is.
