Display more than 10 items in reports

Allow the display of more than 10 items when a report is run. It's really painful to look at a 300+ item report 10 items at a time.

Confirm payouts in payroll report

When marking Paid on the Payroll by Rendering Provider, display a confirmation window so I don't accidentally mark something as paid when it wasn't.
