Display supervisor and supervisee on superbills

Allow me the option to include the name of my supervisor and supervisee on superbills because our state board requires it.

      It would be amazing if we could have the option to include the Appointment Staff and Supervisor to all billing documents; Invoices, Superbills, and Statements. =) Currently, it's only possible to include the Rendering Provider and Billing Provider.

      In order to follow the LPC board rules, we need to have the supervisor's name to display on a superbill next to an intern's name.
