Display client's self pay amount in Calendar and Agenda

When a client has insurance, the amount of the copay shows up on the calendar next to the client's appointment. This is helpful, so the therapist can see what is owed. However, if the client is self-pay, the fixed sliding scale rate does not show up on the calendar!

      In the same way that I can see a co-pay on the calendar screen in that small gray bubble, being able to indicate the cash rate for session in the same way would be SO helpful! (Maybe in a different color bubble to tell the difference between cash and insurance).

      In the calendar or agenda, where it currently displays the co-pay due for the session, also have it automatically display the out of pocket rate due for the session, when applicable for non-insurance clients.
