Add another assessment and set alerts/reminders

We are required to complete an assessment after a certain amount of time depending on the payer and/or state regulations. Allow me to set a reminder or an alert for when a client needs to have an assessment completed, and allow me to complete another assessment without having to add addendums to the first assessment.

    Allow for the ability to re-assess a client with a new assessment rather than adding an addendum.

    This was suggested 5 years ago, please add this! It is standard best clinical practice to update clinical assessments periodically and at least annually.

    We would love to see something where the system could remind practitioners when an annual review comes due. Something where we could set a date to be remind practitioners when they need to either schedule a new intake for Medicaid clients or add an addendum to the initial assessment stating they reviewed the dx and medical necessity as required by commercial insurance.

    We need the ability to have multiple Initial Assessment and Diagnostic Codes forms per chart. This would help us to easily stay in compliance with 3rd party payers who require this form to be updated every 12 months. It would also allow us to have a unique IADC form for different cases. As of now, if a client comes in for an intake, completes TX, then comes back in again, we are limited to one form.

    We need to be able to complete additional assessments- for example, when a client is transferred or returns to therapy after an absence. Currently, the only options are to un-sign the initial assessment and attempt to update it (which ends up being a confusing, especially when there are multiple providers involved) or try to write a detailed assessment in an addendum. Please add this feature theranest!

    Please allow for multiple initial assessments to be completed. This would be much simpler and cleaner to keep all information in one file rather than having to open new cases every time a client returns to services after a time or transitions to another therapist that may want to also complete their own assessment.

    Please add this option to update an assessment in the same way we can update a treatment plan! As others have noted it is required by insurance companies

    Add a review button on the initial assessment report, much like on the treatment plan, as assessments have to be updated annually for ongoing clients. This review feature would then prompt a notification for providers to complete the updated assessment.

    Having the option to create yearly diagnostic assessments is very important for us. Right now we've created an additional case form to make yearly DA updates (along with addendums if needed) however it's not ideal for all situations. Being able to set a yearly assessment reminder would also be extremely helpful. Thanks for the consideration.

    the initial evaluation is just that, your initial evaluation. So coming to a determination that another diagnosis makes sense does not mean you addend the initial evaluation. You should be able to update the diagnoses that pull through to progress notes and treatment plans in a different way. Something new could have happened and that's why you're updating so it would not make sense to have this be part of the initial evaluation.

    Often, clients are not ready to delve into their mental health issues or something occurs where they need to return for services. If the initial intake/assessment was over two years ago, the clinician needs to be able to create a whole new initial assessment for that client. An addendum is not sufficient. I suggest that you make TheraNest where an additional intake assessment can be completed for returning clients. There is no need for creating a new case, just a new intake

    We absolutely MUST be able to add annual assessments. Insurance companies require this before authorization. Presenting problems change. Diagnosis codes must be updated with justification for the changes. This is imperative to getting paid. As it stands, I have to use the initial assessment and put addendums all over the place.

    There should be a way to set a reminder for each assessment. Our policy is to administer the appropriate standardized assessment to the client every month to cover all insurance requirements. We should be able to set a reminder for that.

    It would be nice to have the option to do multiple assessments in one case, as sometimes it's necessary to do more than one but it's not for a new case.

    Virginia Medicaid requires clients to have an reassessment each year to determine if the client continues to meet medical necessity criteria.

    We need to have an alert for when an updated/annual review is needed, like the treatment plan has. Its required by the state of RI and so we can bill out on it as well.
