When viewing a group's appointment history, show if a group note has been completed and whether it's signed or unsigned.

Add a timestamp to Appointments when status is changed

Add a timestamp to Appointments to track when the appointment status is updated. An example of when the timestamp would be beneficial would be to track when appointments are cancelled to by clients on the same day of their appointment.

Don't retroactively change service fee if updated

If I update the fee for a service type the change should only apply to newly created invoices, not past ones. It's easier to update the fee rather than archive it and create a new one.
