Remove unbillable diagnosis codes after period of time

If a diagnosis code is no longer billable and it's past the date to submit corrected claims, then remove the code from TheraNest so I don't accidentally select it.

Set intake date per case

Allow me to set an "intake date" at the case level rather than it defaulting to the client's intake date.
